    Job Title: Physician/Nurse Practitioner for Hospice Services

    At Interim Healthcare, we are looking for a dedicated professional to fill the role of Physician/Nurse Practitioner for our Hospice Services team. This role involves providing medical direction and care to hospice patients as part of the Interdisciplinary Group (IDG).

    Essential Functions:

    • Oversee physician/Nurse Practitioner services for hospice patients to ensure continuity of medical care and effective management of symptoms.
    • Document patient progress in the end-stage disease in medical records.
    • Participate in the development and review of the IDG Plan of Care.
    • Engage in IDG meetings and patient care decisions.
    • Act as a liaison to community physician/Nurse Practitioners.
    • Other duties as assigned.
    • Minimum Requirements: Advanced degree in nursing, current state license, hospice/palliative experience preferred.

    Skills and Qualifications:

    • Knowledge of hospice care regulations and practices.
    • Effective communication skills with both clinical and non-clinical personnel.
    • Commitment to hospice philosophy and patient care.
    • Demonstrated ability to work collaboratively in a team setting.

    Working Conditions:

    • Typical office work environment.
    • Local travel required for patient visits.

    • Physical requirements include bending, lifting, and on-call hours.
