- Oversee physician/Nurse Practitioner services for hospice patients to ensure continuity of medical care and effective management of symptoms.
- Document patient progress in the end-stage disease in medical records.
- Participate in the development and review of the IDG Plan of Care.
- Engage in IDG meetings and patient care decisions.
- Act as a liaison to community physician/Nurse Practitioners.
- Other duties as assigned.
- Minimum Requirements: Advanced degree in nursing, current state license, hospice/palliative experience preferred.
- Knowledge of hospice care regulations and practices.
- Effective communication skills with both clinical and non-clinical personnel.
- Commitment to hospice philosophy and patient care.
- Demonstrated ability to work collaboratively in a team setting.
- Typical office work environment.
- Local travel required for patient visits.
- Exposure to potential hazards and infectious materials.
- Physical requirements include bending, lifting, and on-call hours.
