• Coordinates team-based care to provide health services to individuals, through effective partnerships with patients, their caregivers,/families, community resources, and their physician.
• Facilities "shared goal model" within and across settings to achieve coordinated high-quality care that is patient - and family-centered.
• Ensures continuity and coordination of care across transitions; identifies barriers and facilitates optimal care delivery, and develops a comprehensive plan to address needs including prevention, self-management, access to community resources and education.

• Associates degree required. Bachelors of Nursing preferred.
• LPN Licensure required.
