Job summary
A Care Coordinator Registered Nurse functions in an ambulatory clinic setting as a member of the clinic's patient care team that includes clinic registered nurses (RN), licensed practice nurses (LPN), medical assistants (MA), clinic assistants, technicians clerical staff physician assistants (PA) nurse practitioners NP pharmacists RPh and physicians. This position collaborates with inpatient teams outpatient teams and the patient and their supports to coordinate a smooth transition from hospital or inpatient stay to home.- Coordinate and facilitate patient centered interdisciplinary care and communication
- Provide regular consistent follow up during transitions of care with patient caregivers other healthcare professionals
