- Offers psychosocial interventions with utmost integrity, considering cultural appropriateness and developmental stages throughout the illness continuum, including:

- guidance for the future
- advocacy
- assistance with family gatherings
- connection to community resources
- non-pharmacological pain and symptom management
- Evaluates risks and intervenes to address potential complications in coping with grief, exploring anticipatory bereavement related to potential death and various aspects of loss and adaptation throughout the illness continuum.
- Provides appropriate care coordination and discharge planning. Assesses team challenges with difficult clinical cases, offering support and debriefing as necessary.
- Visits routinely and unscheduled as a Hospice Interdisciplinary Team member for psychosocial patient/family evaluations. Schedules visits based on patient/family acuity levels. Conducts informational visits as requested by the manager for potential patients/families.
- Completes written psychosocial evaluations of patients and families, conveying psychosocial strengths and weaknesses, coping skills, grief responses, support systems, high-risk factors for complicated bereavement, and the necessity for community resources in a timely manner.
- Offers individual and/or family counseling, reassessing psychosocial factors during each visit to ensure optimal results. Initiates or participates in patient care conferences or family meetings as required, documenting appropriately in the electronic record.
- Following patient/family evaluations, makes all necessary referrals to additional community agencies promptly; these may include accessing financial resources, nursing home placements, or transfers to a hospice facility.
- As a Mandated Reporter, possesses knowledge of family violence, child/elder/disabled neglect and abuse to identify at-risk individuals and refer them to suitable organizations under the guidance of the Manager/Director.
- Leads or co-leads monthly support groups for IDT and assists in providing Bereavement Group support, if needed.
- Participates in weekly IDT meetings for ongoing updates, planning, and adjustment of the Plan of Care based on evolving patient/family needs, documented every two weeks. Engages in orientation, education, in-service training, and the Hospice QAPI program.
- Offers consultation to other healthcare professionals regarding psychosocial matters and delivers in-service education when requested.
- Clinical visit notes support the necessity for hospice medical social services considering the patient's overall medical condition and experiences, the intricacy of the services performed, and the plan for the following visit based on prior outcomes.
- The initial evaluation must be finalized within five days of admission.
- Records information in accordance with agency timeliness regulations.
- Completes IDT notes promptly, including risk evaluations and follow-up plans.
- Holds a Master's degree in Social Work (MSW) from an accredited social work school.
- Carries a current LCSW license to practice in Massachusetts.
- Preferred current Massachusetts Licensed Independent Clinical Social Worker (LICSW).
- Maintains certification in American Heart Association (AHA) CPR.
- Minimum of three years of experience in Medical Social Work.
- At least one year of experience in Palliative or End-of-Life Care.
