    Benchmarks Supported Living program assists adults with daily activity skills to gain independence in small personal home settings. Our diverse and dynamic programs cant operate efficiently without the strength and heart of our nurses

    Benchmark is seeking an RN Healthcare Coordinator to provide healthcare coordination and direct intervention for individuals in our Supportive Living and Adult Day Service programs. The RN will provide training for unlicensed staff and contribute to the clinical oversight of program LPNs.

    RESPONSIBILITIES:

    • Provide healthcare and wellness coordination for individuals in accordance with all county, state and federal regulations.
    • Function as liaison and advocate for individuals healthcare with all members of individual medical and interdisciplinary teams.
    • Train unlicensed staff on medication administration, delegated tasks, treatment protocols, and monitoring.
    • Implement and train employees on new and revised health related policies and procedures.
    • Work with Program Manager(s) in overseeing pharmacy and medication needs.
    • Maintain health-related records, complete documentation and assessments.
    • Oversee that the health status and physical conditions of individuals are assessed, monitored, and reported.
    • Review and work with Program Coordinators as needed to develop and update health risk plans.
    • Compile and submit monthly Nursing Report to the Manager of Health Services.
    • Manage transitions of care to and from hospital, nursing home, rehab facility and/or another agency.
    • Respond to emergency medical situations in a timely manner, as appropriate.
    • Mandatory reporter of any suspected abuse, neglect, or exploitation.

    QUALIFICATIONS:

    • RN with current Indiana license.
    • experience working with individuals with IDD
    • Valid Drivers License
