Welcome We're excited you're considering an opportunity with us To apply to this position and be considered click the Apply button located above this message and complete the application in full Below you'll find other important information about this position Responsible for the direct and indirect nursing care of the patients in the home In collaboration with the patient family and health care team IDT the registered nurse as case manager will develop implement and evaluate nursing care according to agency and system policies Provides a safe comfortable environment inthe patient's home Ensures privacy demonstrates concern forthe dignity of patient in performing all patient care Completes assignments in a timely manner Assumes on-call responsibility per policy procedures Communicates patient care problems potential problems to Administrative Staff others IDT members as identified/occur assures continuity coordination of care Assists patients families caregivers providers educational training activities related hospice service current disease processes treatment options medications prescribed therapy treatments available medical social spiritual services rendered Hospice aide performs basic skilled personal supportive routine maintenance activities necessary promote comfort maintain well-being of terminally ill individual who elects receive palliative services at end life stage
