    Nights: 10pm-6:30am on Friday and Saturday nights only

    Role:


    We are seeking a dedicated Life Services Nurse (Memory Care Nurse) to join our Maggie's Place Memory Care team This position is responsible for the provision of tenant services, including assistance with personal care, medications, social and recreational activities, meal services and other tasks as needed to facilitate the continued well-being of tenants.

    Has the ability to communicate effectively and advocate for resident centered/directed care to team members, families, physicians, allied health professionals and the at-large community.


    Responsibilities:


    Provides services to tenants as assigned and as indicated on Tenant Service Plans which may include grooming and dressing guidance, assistance with bathing, and medication reminders/ assistance.

    Assists with the preparation and service of meals and snacks, followed by clean-up.

    Housekeeping to include dusting and vacuuming, cleaning bathrooms and common spaces; Maintain kitchen cleanliness; Monitor food expiration dates; Make beds and change linens; Maintain laundry service; Take out garbage.

    Maintain activity calendar and engage tenants in activities such as spiritual, educational, games, music, and physical activities; Provides companionship.

    Holds huddles to review the Staff 24-Hour Communication log and corresponding Tenant Service Notes for information about changes in tenant service needs and/or health conditions.

    Ability to observe and recommend assessment if tenant demonstrates a significant change in health or functional status.
    Completes required forms and documents in accordance with company policy and state and/or federal regulations.
    Assist with implementation of Life Safety and emergency policies and procedures.
    Ability to communicate and advocate with physicians and other health professionals regarding tenants health status or health needs.
    Promotes staff performance through education, praising, and recognizing effective performance.
    Provides disciplinary counseling for staff as appropriate with direction from AL Director.
    Performs all other duties as assigned by the AL Director or Executive Director.


    Qualifications:


    The Life Services Nurse must be a graduate from an accredited school, be currently licensed in state of which applying without encumbrance, and be responsible for maintaining professional licensure.

    He/she must have a demonstrated ability to effectively communicate verbally, with the ability to read, write, and understand English.
    Must be CPR certified.
    Must have 1-2 years previous experience working with individuals with dementia, preferably in a memory care setting.
