    Description
    ESSENTIAL RESPONSIBILITIESProvides treatment to residents per the physical therapy plan of care established by the Physical Therapist. Assures treatment programs are modified as needed.
    Screens residents to determine if physical therapy services are warranted.
    Involves residents and their supportive family and friends in goal setting.
    Maintains appropriate documentation including treatment notes, recommendations forms, and communication forms to improve quality of discharge reports.
    Develops and instructs residents, family members/significant others in home programs as needed.

    Develop and maintain rapport and communication with physicians, facility staff, family members/significant others and community agencies to ensure effective exchange of information regarding current and future program planning.

    Facilitates rehabilitation team development and ongoing rapport with physicians, facility staff, family members/significant others and community agencies to ensure effective exchange of information regarding resident's activity level and therapy progress.

    Ensures accurate and timely submission of billing logs.
    Ability to plan, organize, and prioritize work.
    Fluent in maintaining and protecting confidentiality.
    Ensures representation at resident related meetings as assigned.

    Reports inadequate inventory of physical therapy supplies to Rehab Aide and Director of Rehab to help manage availability of necessary items for patient care.

    Supports orientation to physicians, facility personnel and the general public to ensure their understanding of the significance, purpose and proper utilization of physical therapy services.

    Provides leadership in further development/expansion of physical therapy services and projects including the development of short and long-term goals.

    Meets the annual goals and objectives that are consistent with the values and mission of Oakwood Village and continuous quality improvement.

    Maintains knowledge of current reimbursement and regulatory issues regarding rehabilitation.
    Exemplifies professional conduct.
    Attends required in-services and continuing education programs.
    Willingness to work flexible schedule including weekends and holidays as needed. Work hours vary dependent upon patient needs and caseload.
    QUALIFICATIONSAbility to provide inspiring leadership through example, planning, teaching, training and empowerment.


    Excellent communication skills to include:
    written, oral, public speaking, listening and nonverbal. Communication should be in a positive spirit.
    Works independently with little supervision and is sensitive to the needs of many constituents. Uses sound judgment and diplomacy in performance of duties.
    Knowledge of the fundamentals of individual and group activities; occupational, physical and speech therapies with an older adult population.
    Knowledge of the scope of practice for physical therapy.
    Working knowledge of reimbursement sources, Medicare A (PPS), Medicare B, Medicaid and private insurance.
    Able to present physical therapy objectives at general and professional levels.
    Working knowledge of Windows based operating systems.
    Trustworthy, dependable, and ethical with willingness to accept suggestions and the ability to cooperate.
    High degree of personal cleanliness and hygiene, neat and well-groomed in appearance. Reliable in following business casual attire alongside Oakwood dress code policy.
    Exhibits a sincere desire to work with older adults and has a growing understanding of the aging process.
    EXPERIENCE, EDUCATION, AND/OR TRAININGGraduate of an accredited Physical Therapist Assistant program.
    Minimum of two years therapist experience preferred.
    Long term care experience preferred.
    Current licensure as a physical therapist assistant in the state of Wisconsin.
