Primary Care Physician - Internal Medicine or Family Medicine opportunity in Indianapolis.

Excellent opportunity for an IN licensed, BC/BE Primary Care Physician (IM or FM) for this growing value-based medicine practice in the heart of the Midwest - Indianapolis, IN. This fast-growing, senior focused, team approach to primary care provides a positive and well-balanced environment. This model is positioned to provide better outcomes for senior patients as we provide multi-disciplinary services and coordinated care supported by analytics and community relationships.

• Mon - Friday schedule no weekends
• Average 12 patients per day
• Lots of time with patients - hour-long initial visits and typically 45 minute-long appointments

Requirements:

• Board Certified or Board Eligible Internal Medicine, Family Medicine or Geriatrician
• Active and unrestricted DEA
• Knowledge of Medicare guidelines helpful
• Knowledge of HEDIS quality indicators helpful
