The Outpatient Coder/Abstractor Level II is responsible for coding and abstracting medical records in accordance with established guidelines for outpatient hospital services.This includes same-day surgery observation emergency department services clinic services infusion center services diagnostic testing under WPH Performance Standards policies and behaviors accurately assign codes meet established coding guidelines ICD-10 CM CPT-4 HCPCS Modifiers analyze medical records identify all appropriate coding sequencing diagnoses procedures review local coverage determinations National Coverage Determination alerts ensure diagnoses accurately captured documented seek clarification information needed assign accurate specific codes alert physician additional supporting documentation needed limited charge correction charge validation effective communication departments essential proficiency spreadsheets required individual demonstrate ability work independently query physicians providers clarification documentation improvement when needed abstract required data health information system ensuring accuracy fields meets quality standards coding abstracting medical records meets productivity standards completes work within time frames stays update industry changes HIM best practices payer-specific guidelines uses Microsoft applications HIM software applications including but not limited to 3M 360 coding applications Optum Lynx attends staff meetings updated knowledge current department hospital issues status attends educational meetings requested updated knowledge related coding compliance completes annual competency requirements performs related duties assigned.
