    Diadem Hearts provides HCBS-AMH (Home and Community Based Services-Adult Mental Health) services for people with serious mental illness. Our target population is Long-Term Psychiatric hospitalization, Jail Diversion, and Emergency Department Diversion. Diadem Hearts' mission is to enable individuals to achieve the highest level of potential in their day-to-day independent activities to help them experience successful tenure in their community.

    Referral Coordinator

    Performs a wide range of administrative duties to include referral processing, verification of benefits, eligibility verifications, communication to potential candidates, promote HBCS-AMH program to potential referral sources as well as maintaining communication with referral sources and other tasks assigned.

    Skills and Qualifications:

    • Responsible for the tracking, coordinating, and communication of patient referrals.
    • Ensures complete and accurate patient registration, including demographics, current insurance information and supporting enrollment documentation.
    • Handles requests from third-party referral sources and provides relevant information and data.
    • Assembles information concerning patients' clinical backgrounds and referral needs.
    • Reviews details and expectations of referral with the potential patient
    • Assists patients in problem-solving potential issues related to barriers during the enrollment process.
    • Ensures that referrals are addressed in a timely manner.
    • Communication with referral sources to ensure patient information/ registration is complete.
    • Communicates patient updates between the program and referring source.
    • Promote and market HCBS-AMH program with potential referral sources.

    Qualifications and Education Requirements

    • Bachelor's degree or two years of relevant experience.
    • Strong customer service focus.
    • Effective verbal and written communication skills.
    • Teamwork orientation
    • Organized and able to manage multiple priorities.
    • Good judgment.
    • Resourcefulness in problem-solving.
    • Able to take and follow through with delegated tasks and accountability.

    Additional requirements:

    • Driver's license
    • Ability to pass background checks.
