Job DescriptionCASE MANAGER
BASIC FUNCTION:
The basic function of the Case Manager is to establish and build supportive and nurturing relationships with the child. To coordinate all services to meet the child's needs and act as a liaison between children, staff, managing conservator, and all service providers
JOB RESPONSIBILITY
-Actively participates in treatment team meetings and special staffing as needed
-Prepares and implements individualized service plans
-Attends Caseworker meetings
-Coordinate PAL and other outdoor programs for Children
-Discharge Planning & Implementation
-Maintain Population report
Executing Intake Process and coordination with Child's caseworker for process implementation.
-Timely completion of Admission assessment, ISP, and Treatment goal updates.
-Audits children's files regarding Service Plans as needed for compliance with residential child-care licensing, residential contracts, agency policies and procedures, and Youth for Tomorrow (YFT) standards
Communicates and reviews the overall Child's Performance with the Treatment program, Administrator, and Management.
follow requirements and checklist for Heightened Monitoring, CAP, YFT Audit, Contracts, SFM, and any other contract providers.
Develops and implements new processes and forms which assist in meeting Compliance standards.
Participates in in-service training
Review daily logs, incident & serious incident reports, visual checks, case notes, and safety plans within 24 hours of occurrence.
Maintain a supportive relationship with resources which includes identifying ongoing training and support needs and assisting in obtaining such services along with other team members.
Recommends staff training to enhance the ongoing development of the treatment team.
Participates in public relations assignments and meetings as needed
As needed, be available to the treatment team 24 hours a day to assist in crisis intervention and to provide assistance for treatment issues during a crisis.
Performs other assignments and duties as necessary and the Executive Director's requests.
Coordinate Outdoor activities & Community Events for Children
-Ability to effectively work under pressure and remain flexible as priorities change.
-Must be able to pass a Fingerprint-Based criminal history check and a DFPS central registry check before you are hired.
-Must qualify as a child-placement management staff according to Minimum Standards for Group Residential Operations
-Weekly Caseworker meetings & updates on each Child via email or Telephone
Daily & weekly review of therapy notes, and if a change is required for the Treatment plan as reflected in the sessions, notify the treatment team immediately on changes and require updates and notify caseworkers and treatment teams.
Monitor and update current Safety Plans, reflect on the treatment, and notify caseworkers as needed.
Document Red flag meetings & follow up on the decisions.
-During any Serious Incidents, the Child`s Caseworker should be notified immediately via email or Telephone
-Attending School meetings on behalf of the Child, if necessary (Backup role)
-School communication on behalf of the Child
-Updating Caseworker on any incidents. (During any incidents at School, the reports should be followed up with the School authorities, documented in the Child folder, and updated with the Caseworker for the Child. (Backup Role)
-All appointments must be within the timeline specified by minimum standards.
-Updating Child Folder daily
-Monitor appointments with the Physician's office, psychiatrist, dentist, and any other requirements (Backup role), which should meet minimum standards for each Child.
-Filing and reviewing documents timely.
-YFT level care assessment to be sent out to YFT Caseworker
-Check Child's folder for filing and update the Administrator or Office in Charge of missing documents.
-Updating the Child caseworker for any incidents, hospital visits, or changes of meds
-Collecting and coordinating Hospital records for admission and discharge.
-As needed, coordinate with Court, Caseworker for child visit (Virtual, Physical location)
-Facility Inspection on Holes and repairs (Need to be updated to Administrator)
-Flexibility with Shift/Days
-Prepare all the documentation for the child's visits to the physician, dentist, ER, and other specialists.
-Communicate to the staff who will transport a child for the appointments.
-Constantly update on a work chat about upcoming appointments, Blood work, and if it requires special attention.
-Send out a notice for weekly schedules, appointments, and visitation.
-Prepare and submit all the relevant documentation for a psychotropic medication review to the medical coordinator. (Blood work, serious injuries, hospitalization, hospital and doctors' notes).
-Send all the medication changes to the caseworker within 24-48 hours.
At-Beginning of the month, submit all the Therapy notes to the caseworkers.
-Please cc all the communication to the Administrator.
-At the end of the day, please submit a brief daily work report to the Administrator & Executive Director. Especially on any changes to the scheduled events.
-Other duties as needed.
MINIMUM QUALIFICATIONS:
This position requires a person with a Bachelor's degree, preferably in a human service-related field. Two years of experience working for a General Residential Operation or other related job experience is required.
