- Home visits - Housed clients: minimum of two contacts monthly including face-to-face visits at home. Unhoused clients: during the first 90 days in the program, at least four contacts monthly including face-to-face community visits and regular communication via phone, email, text, or in person. For participants over 90 days in the program, two home visits and one community or office visit are required.
- Case Management - Identify critical issues, barriers, needs, and strengths of clients. Develop goals, connect with necessary resources, and provide support to help them achieve their objectives.
- Engage clients, explain services, conduct program intake, and assess housing barriers, needs, income, and preferences.
- Create service plans with participants to secure safe, affordable housing, and facilitate connections to referrals and community resources.
- Assist with outreach and negotiations with landlords, as well as rental and job applications.

Description
Join our team at XYZ Company as a Case Manager! You will work closely with our Program Manager to provide top-notch services for our participants. Your responsibilities will include conducting participant assessments, offering solution-focused family centered case management, coordinating relocation when necessary, organizing Healing Circles, enrolling individuals and families in support services, and keeping detailed case notes in our ClientTrack software.
PRINCIPLE ACCOUNTABILITIES:
Bachelor's degree in social work, psychology, sociology, counseling, or related field with at least one year of experience in case management.
Experience
Minimum of two years' experience in homeless programs or social services. Knowledge of community resources and excellent communication skills are essential.
Additional Requirements:
Applicants must pass drug and TB testing, background checks, and Child Protective Services clearance. COVID vaccination is mandatory.
