The Registered Nurse (RN) in Home Health delivers and oversees nursing care for patients at their residences as per physician's orders and in line with regulations, laws, and agency protocols.
They collaborate with other healthcare professionals to plan the overall care, ensuring continuity through effective communication with physicians, colleagues, patients, families, and community resources, all within a Point of Care setting.
The RN conducts initial and/or comprehensive nursing assessments, sets care priorities, creates care plans as per state regulations, and maintains accurate, thorough, and timely documentation aligned with physician instructions.
They perform comprehensive patient assessments at specific intervals and establish suitable care plans. Moreover, they verify that patients meet the eligibility criteria and medical necessity standards for home health services.
