    Responsibilities:

    The In-Home Services Records Cooridnator will manage medical records documentation between the In-Home Services EMR platform and InnovAge PACE EMR. Additionally, they will be competent in functioning within the home care EMR to assist and complete scheduling tasks, verify visits, documentation, and complete audits to identify compliance. Finally, they will run, compile, and pull reports to oversee and manage compliance dates for documentation, assessments, specified home care visit types. Additionally, they provide back-up support to other home care activities for scheduling and administrative tasks.

    Participant Records 60%

    • Prepares charts for new participants and ensures the correctness of information received.
    • Updates systems to ensure accurate demographics.
    • Maintains the active, closed and purged records for all participants.
    • Files documents into participant records on a timely basis.
    • Scans documents into participant records on a timely basis.
    • Maintains and monitors assessment and reassessment schedules for participants.
    • Records and compiles monthly census and reports.
    • Monitor the care of InnovAge PACE I.H.S program Participants.
    • Communicate and build relationships with contract I.H.S entities and providers to garner documents and communication timely.
    • Manage Participant file in both I.H.S EMR and PACE EMR for completeness and compliance.
    • Additional work functions and duties as assigned

    Scheduling & Operations 40%

    • Answering In Home Services incoming phone calls.
    • Entering and managing Participant schedules per standard operating procedures.
    • Managing compliance check points within Participant services and schedules.
    • Managing In Home Services orders in relation to scheduled services.
    • Coordinate care between Participants and assigned home care field staff to support completion of assigned and necessary services.
    • Effectively communicate and build relationships with field staff to assist in scheduling and covering available shifts
    • Review and verify completion of shifts and visits to include documentation completion in anticipation of records coordination.
    • Additional Scheduling and Operations functions as assigned.

    REQUIRED

    • 2+ year in Medical Records
    • Current knowledge of medical records documentation requirements for long-term care and/or Home Health Services.
    • High School Diploma or GED

    PREFERRED

    • Associates or Bachelors in Health Information Technology or Business Administration
    • A minimum of one (1) year experience in home health and/or a home care setting
    • Completion of course work for accredited records technician
    • Accredited Records Technician Certification
    • RHIT or RHIA
    • Experience in scheduling patient appointments and follow-up.
    • Bilingual

    Salary ranges are dependent on a variety of factors, including qualifications, experience, and geographic location and does not include potential bonus or benefits.
