Position Summary: We are seeking a dedicated Physical Therapist (Full-Time) to join our rehabilitation team. The ideal candidate will provide high-quality, evidence-based care to residents across a variety of care settings. You will work closely with our interdisciplinary team to assess, plan, and implement individualized treatment plans that improve mobility, strength, and overall quality of life.Key Responsibilities:
- Evaluate and treat residents to improve functional mobility, balance, strength, and endurance.
- Develop and implement individualized care plans based on assessment findings.
- Monitor and document patient progress, adjusting treatment plans as necessary.
- Educate residents and families on exercises, safety, and health management.
- Collaborate with nurses, occupational therapists, speech therapists, and other team members to provide comprehensive care.
- Assist with discharge planning and support residents in returning home whenever possible.
- Participate in staff meetings, in-services, and continuing education opportunities.
- Doctor of Physical Therapy (DPT) and current Nebraska Physical Therapy license.

- Strong interpersonal, communication, and documentation skills.
- Ability to work independently and as part of a collaborative team.
- Experience in long-term care or short-stay rehab is a plus.
