Description
In this role, you will be a member of our collaborative team of Physicians and Providers working to provide patient and family centered care throughout the practice.
You will assess, plan, evaluate, implement and coordinate the care of patients, often times in collaboration with the CCHVC physicians.
Full time, 1.Day shift - Rotating evening until 8:00pm about once every two weeks.Rotating weekends and holidays - day shift about once every 2-3 months.
