    The Bedford-Stuyvesant Family Health Center (BSFHC) is a patient-centered, team based multi-specialty practice in central Brooklyn. BSFHC has been providing affordable, comprehensive, outpatient medical and dental services in an outpatient setting for over 40 years. As a Federally Qualified Health Center (FQHC), BSFHC serves all of the primary health care needs of families in the heart of North and Central Brooklyn. Our mission is to provide the most professional, courteous, and highest quality health care, with dignity, to those we serve, especially the undeserved population, without regard for ability to pay.

    We are seeking a Patient Advocate to assesses member needs and identifies solutions and link all eligible clients to community-based services designed to enhance their lives. Patient Advocate assisting clients and their families in the process of obtaining maximum benefits available for medical, financial entitlement and employment, which support the physical, mental and psychosocial well-being of each client and family.

    Duties and Responsibilities include but not limited to:
    • Completing initial intake interviews with new clients and conducting thorough needs assessments to determine program eligibility.
    • Working with clients to establish services plans leading to crisis resolution and self-sufficiency.
    • Providing clients with employment services and/or referrals for employment, training, vocational training, and education. This includes resume and cover letter writing, mock interviews, referral to professional services for employment purposes.
    • Advocating for and assisting clients with the service delivery system to ensure receipt of entitlements and housing stabilization.
    • Preparing thorough, accurate, and complete program financial assistance requests, as needed, to assist with resolving clients' housing crises.
    • Provide relocation housing assistance; assist clients through affordable housing process, assisted living and educated clients on other methods of searching for housing.
    • Educate clients about land lord/tenant rights, inform/refer clients for available housing vouchers and refer to legal services.
    • Complete and submit SNAP applications; guide individuals on the Public Assistance protocol
    • Register clients for health insurance: Medicaid, Medicare, and/or private insurance
    • Educate and assist clients applying for Social Security/ Social Security Disability
    • Serve as a liaison between intra-departments that include nursing, medical records, patient relations and the patients/family and other service entities among the continuum of care
    • Input client data and client progress information into automated database.
    Education, Licenses and/or Certifications, Experience Requirements:
    • High School/ GED or some college. Bachelor's Degree in Social Work or a related field preferred.
    • A minimum of two years' experience in case management, assessment, counseling and crisis intervention preferred.
    • Strong verbal, written, and negotiation skills.
