    The Registered Nurse (RN) is responsible for overseeing both clinical and direct services provided by a home health care agency, encompassing both medical and non-medical care. This role involves assessing patient needs, wellness visits, and in home services. The (RN) also plays a vital role in evaluating patients for home health appropriateness, developing care plans in collaboration with physicians, and educating patients, families, caregivers, and community providers. Monitoring and assessing the effectiveness of home care plans and patient responses are essential aspects of this role.

    Key Responsibilities:

    Clinical and Staff Management:

    • Coordinate patient-care activities and Care Plans with physicians and providers.
    • Conduct home visits to manage care plans in accordance with physician orders.
    • Clinical Documentation: Ensuring accurate and thorough clinical documentation.
    • Reviewing Visit Notes: Regularly reviewing visit notes from staff to ensure proper care is being completed by clinical and direct care staff.

    Compliance and Record-Keeping:

    • Ensure proper performance and utilization of EVV systems to comply with Pennsylvania state or payor regulations.
    • Stay current with state and federal governmental requirements.
    • Maintain awareness of the Individual Rights and Responsibility Act.
    • Adhere to OASIS regulations for quality care.
    • Complete disciplinary action forms and performance evaluations for clinical and direct care staff supervision.

    Team Leadership:

    • Maintain regular communication through phone calls, one-on-one supervision, team meetings, and quarterly evaluations.

    Collaboration and Reporting:

    • Collaborate closely with the VP for the first six months, with regular check-ins with senior leadership.
    • Report progress on the development of the Philadelphia market and Medicare processes to the COO, President, CEO, VP, and CCO.

    Schedule:

    • The RN will work a minimum of 15 hours and maximum of 20 hours per week providing direct care services and administrative tasks in our Philadelphia office.

    Job Specification: Registered Nurses (RN)

    To qualify for the Registered Nurses (RN) position, candidates must meet the following requirements:

    1. Valid Pennsylvania RN License: Candidates must possess an active and valid Pennsylvania Registered Nurse (RN) license.
    2. BSN Preferred: While not mandatory, a Bachelor of Science in Nursing (BSN) is preferred.
    3. Minimum One (1) Year Relevant Clinical RN Experience: Candidates should have a minimum of one year of relevant clinical RN experience.
    4. CPR Certified: Candidates must hold current CPR certification.
    5. Recent, Relevant Experience in a Medicare-Certified Home Health Agency: Ideal candidates should have recent and relevant experience working in a Medicare-certified home health agency in roles such as a case manager, primary nurse, or as a BSN student.
