- Navigating electronic health records, documenting progress notes, and formal communications.
- Monitoring pharmacy care plans during the treatment process.
- Conducting medication reconciliation and necessary interventions.
- Establishing professional relationships with patients and the care team.
- Providing drug information to the healthcare team and counseling patients or caregivers accordingly.
- Ensuring safe selection of infusion supplies.
- Collaborating with the team to determine suitable drug administration methods.
- Managing aminoglycoside and vancomycin therapies.
- Communicating clinical updates to referring providers.
- Coordinating TPN management with the home infusion team.
- Overseeing the quality of sterile preparations.
- Assisting with device troubleshooting for patients, caregivers, and healthcare providers.
- Working collaboratively with technicians and team members to optimize work efficiency.
- Participating actively in team meetings and other relevant initiatives.
- Providing on-call support as per the rotating schedule.
- Bachelor's degree in Pharmacy.
- Graduate from an ACPE-accredited School of Pharmacy.
- Active IL Pharmacist license in good standing.
- Minimum of 2 years of acute care or home infusion experience.
- Doctorate of Pharmacy.
- Completion of ASHP-accredited Pharmacy Practice Residency Program.
- Board Certification (e.g., BCPS, BCACP, BCGP).
- MTM certification (e.g., APhA).
