    **Job Details:**

    This clinical pharmacy specialist is responsible for medication management for patients treated in a primary care or a medical, surgical, or pediatric specialty clinic.

    They collaborate with other healthcare professionals to design, implement, monitor, evaluate and modify pharmacotherapy to ensure safe, effective and economical patient care.

    The pharmacist identifies potential or actual adverse events resulting from drug therapy and recommends or initiates the necessary treatment.

    They identify barriers to medication adherence incorporating patient characteristics such as cultural, religious and personal beliefs, socioeconomic status, healthcare literacy and logistics.

    The pharmacist works closely with the care team and patient to improve access to and / or modify drug therapy to overcome barriers.

    Clinical pharmacy specialists provide leadership by facilitating the development, implementation and maintenance of systems that promote desirable patient outcomes.

    They educate professional staff members, students, interns and residents.

    They initiate and participate in drug therapy research.

    **Job Qualifications:**

    Bachelor's degree in Pharmacy or a PharmD degree from an accredited college or university is required.

    Current MA license to practice pharmacy or eligibility for licensure required.
