Location: Sylmar, CA

Schedule: 8-hour shifts. Must be flexible to work all shifts (mainly 1st and 2nd- an occasional 3rd shift may be required)

Job Responsibilities:

• Participates in the development and continual monitoring of patient drug therapy treatment plans and patient outes in clinical specialty programs
• Performs patient assessments and monitors the effects of drug therapy using clinical symptoms, evaluating laboratory data, and other relevant clinical diagnostic techniques
• Attends daily rounds and medical conferences with physicians in specialty medical areas to evaluate patient drug therapy regimens
• Interview patients to obtain information regarding drug use, drug allergies and sensitivities, and medication histories; assesses pliance and counsel patients on appropriate drug use
• Participates in ongoing medication usage evaluating programs by assisting in the establishment of criteria for cost effectiveness/control utilization use, process indicators, and monitoring parameters
• Conducts evaluations to determine pliance to the established medication use evaluation criteria
• Participates in the establishment of critical pathways to ensure optimal medication usage
• Critically analyzes, evaluates, and prepares remendations of medical literature in relation to medication therapy
• Interprets and applies pharmacokinetic drug data in remendations for patient specific drug dosage regimen
• Advises and assists in implementing cost containment strategies for the usage of pharmaceutical products
• Participates in performance improvement activities to ensure optimal pharmaceutical care

Ideal Candidate:

• PGY-1 or 2 years of current inpatient experience
• Must be fully COVID-19 Vaccinated
• Full time availability
• Registered Pharmacist in the state of California
