    We are currently seeking home providers in Iowa who can offer a meaningful work opportunity from their own homes.

    As a Host Home / Shared Living provider with Mosaic at Home, you can earn an average annual income of $80,000 (tax-free) plus room and board.

    Mosaic has been providing improved quality of life, greater stability, and increased community access for people with disabilities through our shared living service for nearly 40 years.

    Your Role as a Home Provider

    As a home provider, you will share your home with an individual with a disability. Your primary responsibility is to maintain a safe and healthy home environment for the individual and serve as a mentor while assisting them with daily tasks as needed.

    You will help cultivate life skills and promote independence by participating in activities of daily living together, both at home and in the community.

    The matching process allows you to match with an individual whose needs align with the support level you wish to offer. Duties may vary from minimal support (budgeting, medication, transportation, etc.) to full support (dressing, toileting, feeding, etc.), and the income is commensurate with the level of support provided.

    Key Responsibilities:

    • Provide support for daily tasks such as cooking, cleaning, laundry, budgeting, decision-making, etc.
    • Skill acquisition and development
    • Medication administration and management
    • Accompanying individuals to medical appointments
    • Supporting overall health and well-being
    • Daily documentation via web-based case management application
    • Planning fun and leisure experiences for individuals, promoting community inclusion, integration, and socialization
    • Compliance with state and agency policies and procedures
    • Effective communication with team members as needed

    Possible Additional Responsibilities:

    • Assistance with personal care tasks like bathing, toileting, and feeding
    • Providing support for medical protocols, such as seizure response or fall prevention
    • Caregiving for individuals with complex physical or medical needs
    • Supporting individuals with behavioral issues

    Requirements:

    • You must be a resident of the state where you wish to provide contracted services
    • Valid driver's license, car insurance, and current registration
    • Active homeowners or renters insurance
    • You must own or rent your own home/apartment/condo/townhouse
    • You must be at least 18 years old
    • Experience is preferred but not required
    • Reliable computer and internet access
    • Completion of all state-mandated trainings
    • A safe and clean residence that meets environmental standards
    • Ability to pass State/Federal and APS/CPS background checks
