    Description

    The Nurse Practitioner works in collaboration with the Medical Director and multi-disciplinary team to deliver comprehensive and compassionate hospice and palliative care and services to patients diagnosed with terminal or life-limiting illness in all settings.

    Provides face to face visits, rounding in inpatient centers, and symptom management visits.


    Key Responsibilities:
    Conduct comprehensive assessments of patients, diagnose and manage symptoms.
    Develop individualized care plans in collaboration with the interdisciplinary team, patients, and their families. Establish goals of care based on patient preferences, values, and treatment options.
    Prescribe and manage medications to alleviate symptoms and improve the patient's comfort.
    Communicate effectively with patients, families, and other healthcare team members.
    Provide education on the nature of the illness, treatment options, and end-of-life care decisions.
    Maintain accurate and thorough medical records, documenting assessments, care plans, and interventions. Ensure compliance with regulatory standards and guidelines.

    Shift time:
    Monday-Friday, 8 am to 4:30 pm, with evening and weekend on call responsibility as scheduled.

    Strong clinical assessment and diagnostic skills.
    Excellent communication and interpersonal skills.
    Compassion and empathy in dealing with patients and their families.
    Ability to work collaboratively in a multidisciplinary team.
    Knowledge of ethical issues related to end-of-life and palliative care.

    Graduate of an accredited School of Nursing, completion of post-basic program for advanced nursing practice and board certification
    Current RN/APRN licensure and DEA licensure in Ohio and Kentucky
    Knowledge of current trends and techniques in nursing practice
    At least one year of experience in a patient care position in a hospital, nursing home, or related health care facility preferred
