SummaryThe Inpatient Care Coordinator/Discharge Planner (ICC/DP) Registered Nurse (RN) will coordinate and deliver patient-driven evidenced based care. This RN coordinates care to veterans in the inpatient clinical setting.
Duties
The Inpatient Care Coordinator/Discharge Planner (ICC/DP) Registered Nurse (RN) will coordinate and deliver patient-driven evidenced based care within Nursing Service.
The ICC/DP RN will provide coordinated care to veterans in the inpatient clinical settings.The RN will monitor the patient labs, medications, and activity levels of each admission within 24 hours to evaluate the discharge planning needs.
The RN will support the entire team in delivering quality care in a considerate, professional, and respectful manner.The ICC/DP RN focuses on an Interdisciplinary Team facilitating safe and effective patient discharges from the hospital to the next level of care.
The goal addresses the medical, nursing, and/or personal care needs of Veterans to enable their return to an optimal level of functioning.
The Inpatient Care Coordinator/Discharge Planner RN has an accepted and recognized role in the coordination of care required by patients with catastrophic, complex high-risk, or high-cost health care issues.
They work closely with hospitalist, social work, physical therapy, occupational therapy, community care, palliative care, and pharmacist (hereafter known as the Interdisciplinary Team, IDT) to provide clinical support and knowledge regarding patients being discharged into community health care settings.
The Inpatient Care Coordinator/Discharge Planner role will allow Registered Nurse to initiate prosthetic orders and consultation request as described by the provider.
The RN (a)Collaborates with others in organizing delivery of care to assure continuity of care and peer accountability for practice, including access to care and discharge planning:
(b) Uses their acquired clinical knowledge/judgment to promote peer involvement in planning, decision-making, and evaluating outcomes, (c) Functions as a proficient nurse in clinical practice and/or areas related to their assigned roles and responsibilities, (d) Systematically evaluating current practice, and formulating outcomes for patients and/or organizational processes within the medical center, (e) Guiding, developing, and supporting staff from a management perspective, and (f) Using professional standards of care, scientific evidence, and practice to evaluate service activities within their area.
