    Job Title: Housing Stabilization Services Worker

    Description

    The Housing Stabilization Services Worker provides essential services to support individuals with disabilities, such as mental illness or chemical dependency, in transitioning into stable housing and avoiding future homelessness or institutionalization.

    Key Responsibilities:

    • Provide community-based Housing Stabilization Services to eligible adult clients with a disability that impacts their ability to live successfully in the community.
    • Maintain a caseload of clients and meet with individuals 2-3 hours a week in the community or in the individual's home.
    • Ensure services are connected to the person-centered housing plan developed with each client.
    • Identify and assist with resolving barriers to accessing housing.
    • Support clients in applying for benefits to afford housing and research prospective housing options.
    • Help clients understand and negotiate leases, ensuring safe living arrangements.
    • Identify resources to cover moving expenses and access household goods.

    Qualifications:

    • Bachelor's degree in social sciences (e.g., Social Work, Psychology, Human Services) or equivalent experience working in a community setting providing similar services.
    • Valid Minnesota driver's license, reliable transportation, and valid insurance.

    Work Environment:

    This role involves working in an office setting, remotely from the employee's home, in the community, and within clients' homes. The work environment may involve low to moderate noise levels, frequent standing, walking, sitting, bending, twisting, talking, and hearing.
