    Certified Nursing Assistant (CNA)

    Join our team of dedicated healthcare professionals at StoryPoint of Fort Wayne West. We are seeking a licensed Certified Nursing Assistant (CNA) to provide exceptional care to our residents.

    Responsibilities:

    • Provide 1440 care to residents, leading and assisting as needed.
    • Review Resident Care Plans at the start of each shift to ensure personalized care.
    • Bathe and clean residents by bed bath, shower, and/or whirlpool, maintaining their dignity and comfort.
    • Assist with daily activities such as cleaning teeth, shampooing/combing hair, cleaning and cutting nails, applying make-up, shaving, and cleaning glasses.
    • Help residents dress appropriately, encouraging them to wear usual and comfortable clothing.
    • Make beds, change linens, and maintain a clean and organized room, ensuring a safe environment for residents.
    • Feed residents requiring total or partial assistance with meals, setting up trays and passing snacks.
    • Answer calls lights promptly and ensure they are always within reach.
    • Assist residents with bedpan, urinal, and bedside commode use, or transport them to the bathroom as needed.
    • Take and record vital signs (temperature, pulse, respiration, blood pressure), heights, and weights as requested, reporting any abnormalities to the charge nurse.
    • Measure intake and output on all residents who require it, including emptying urine from indwelling catheters or other collection devices near the end of the shift.
    • Pass water to residents and collect water pitchers for cleaning per schedule.
    • Clean wheelchairs, walkers, commodes, bedpans, and urinals according to the schedule.
    • Report unusual colors or odors of excretes before discarding them.
    • Assist with transporting, transferring, ambulating, frequent positioning, and providing Range of Motion (ROM) to residents, prioritizing their safety and well-being.
    • Maintain personal dignity by ensuring privacy and treating residents with respect, adhering to the Resident Bill of Rights.
    • Apply safety devices as directed by the nurse and check them every 30 minutes, releasing them every 2 hours.
    • Regularly check on residents who cannot call for help.
    • Prepare rooms for new admissions and assist with resident transfers, discharges, and admissions, ensuring a smooth transition.
    • Greet newly admitted residents and escort them to their room as necessary.
    • Complete Nurse Aide flow sheets during shifts and document care given in CareTracker, using only authorized abbreviations.
    • Identify residents before providing care and follow universal precautions and isolation procedures for infection control.
    • Inventorize and mark resident's personal possessions as instructed.
    • Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals immediately.
    • Maintain service areas in a clean and orderly condition and keep hallways clear of equipment.
    • Report accidents or incidents observed immediately.
    • Report suspected abuse immediately.
    • Perform other duties as assigned.

    Accountabilities:

    • Display professionalism in attitude, demeanor, and dress.
    • Show good organizational skills and ability to work under pressure, meeting deadlines.
    • Be subject to frequent interruptions and varying working hours, including days, evenings, weekends, and holidays as needed.
    • Participate in callback situations during emergency conditions, such as severe weather or evacuation.
    • Engage in continuous physical movement, prioritizing resident safety and well-being.
