    Job Title: Psychiatric Rehabilitation Services Coordinator

    About the Role

    We are seeking a highly motivated and compassionate Psychiatric Rehabilitation Services Coordinator to join our team at Saba Health Care. The successful candidate will possess a bachelor's degree in a human services field, such as sociology, special education, or psychology, and have a minimum of one year of supervised experience in mental health or human services.

    Key Responsibilities

    • Integrate, coordinate, and monitor each resident's psychiatric rehabilitation services program
    • Orient residents to the facility and review treatment plans
    • Prepare and assist residents with active participation in treatment plan reviews
    • Coordinate the delivery of psychiatric rehabilitation services programs
    • Monitor residents' self-directed care and compliance with treatment plans

    Administrative Functions

    • Assist in planning, developing, and implementing psychiatric rehabilitation services programs
    • Develop and implement policies and procedures for identifying social and emotional needs
    • Perform administrative tasks, such as completing forms and reports
    • Arrange services and transportation for residents as needed

    Psychiatric Rehabilitation Functions

    • Facilitate assessment processes for new and re-admitted residents
    • Oversee care plan conferences and write goal statements and discharge plans
    • Provide 1:1 counseling, groups, training, and behavioral intervention
    • Write progress notes and chart audits

    Requirements

    • Bachelor's degree in a human services field
    • Minimum one year of supervised experience in mental health or human services
    • Ability to work cooperatively and harmoniously with various personalities
