    Pharmacist Part Time Days Position Summary

    Assures the appropriate selection, dosing, administering, and dispensing of medication orders.

    Responsibilities

    Reviews and assesses medication orders to identify, prevent and resolve potential and actual drug related problems.

    Recommends formulary alternatives when appropriate.

    Compounds and dispenses medications for accurate delivery to patients.

    Provides drug information and education to medical staff and patients.

    Communicates with physicians and other clinicians as necessary to facilitate care.

    Accurately enters/verifies medication orders and performs final check prior to dispensing of all medications, including sterile products, chemotherapy, TPN's, and controlled substances.

    Performs and documents clinical activities and interventions accurately.

    Supervises and directs pharmacy support personnel to provide timely, comprehensive, and cost effective pharmaceutical services.

    Participates in hospital safety/quality programs and assists in training new staff and students.

    Responds to code arrests and other emergent situations where pharmacist assistance is needed.

    Performs essential duties of pharmacy manager or director in his/her absence when the pharmacy clinical coordinator is not available.

    May be required to work at other Carondelet network facilities.

    THE PHARMACIST PART TIME DAYS CANDIDATE WILL POSSESS THE FOLLOWING EDUCATION, LICENSE/CERTIFICATIONS, AND EXPERIENCE.

    LICENSURE/CERTIFICATION/REGISTRATION

    License Required: Valid current Arizona Registered Pharmacist License.

    Certification Required: Basic Life Support (BLS-CPR) upon employment and Advanced Cardiac Life Support (ACLS) within 6 months of employment.

    Certification Required: Board Certification is required for any new hires after January 1st, 2014. Must be board certified within 36 months of eligibility for certification.

    Certification Preferred: Pediatric Advanced Life Support (PALS).

    EDUCATION

    Required: Graduation from an accredited School or College of Pharmacy with a . or PharmD degree.

    WORK EXPERIENCE

    Required: Must be able to work variable shifts including nights, weekends, and holidays.

    Preferred: Residency Preferred, Hospital pharmacy experience, IT Pharmacist with Cerner Millenium

    This individual's responsibilities include (but are not limited to): Review and monitor patient's medication therapy and orders written for safety and efficacy considering specific patient conditions and health status; advise patient care team on patient's medication therapy and document such interventions. Order verification and performing medication profile reviews that should include but not limited to checking for duplications, drug- drug and drug-food interactions, allergies, drug appropriateness, dosing, and frequency. Prepare, label, and dispense medications for patients in accordance with physicians' prescriptions; ensure compliance with Federal, State, and local laws and regulations pertaining to the dispensing of drugs and narcotics and the maintenance of required records. Check High Risk IVs, renal dose adjusting medications, IV to PO conversions, initiates/modifies/ discontinues therapy, and mediates black box warning interventions. Ensure that appropriate medications are available for patients and properly procured, prepared, stored, and maintained. Supervise the preparation, maintenance, and dispensing process of drugs and supplies, including technical personnel performing these functions. Maintain relationships with medical and nursing staff; provide timely information pertaining to pharmaceutical supplies, drug usage and compatibility, state and federal regulations regarding drug controls, and Joint Commission standards. Provide for collaborative practice management as per facility-approved protocols for anticoagulation, parenteral nutrition, vancomycin, and aminoglycoside consults, etc. Assist with medication reconciliation and clarification of unsafe and unclear drug therapy orders. Provide education to patients on the use of medications as needed. Participate in the education and training of healthcare professionals (. physicians, nurses, pharmacy technicians, and students) in relation to medication use. Demonstrate regulatory compliance as evidenced by timely completion of nursing unit inspections, as needed. Maintain professional competence with pharmacy department and state licensing requirements via staff development, independent study, and/or professional activities.

    Education:

    Required: Pharm.
    D. or BS in Pharmacy

    Experience:

    Preferred: Hospital experience

    Certifications:

    Required: Pharmacy license; BLS within 30 days of hire (ACLS and/or PALS if code attendance required);
    Preferred: Board Certification in Pharmacotherapy (BCPS), CGP, etc.

    Physical Demands:

    Lift/position up to 50 lbs. Push/pull up to 50 lbs. of force. Frequent standing and walking. Frequent reaching, stooping, and bending. Manual dexterity, mobility, touch, auditory to perform all the related duties of the position
