    Job Description:

    Home Health Social Worker--Per Diem

    The Home Health Social Worker delivers a variety of services to patients and families while consistently using judgment and discretion to identify their unique spiritual/cultural beliefs, strengths, coping skills, etc. Ongoing assessment is carried out, and the Plan of Care reflects the Medical Social Worker's evaluation and conclusions concerning the distinct bio-psychosocial requirements of the patient/family.

    Job Description:

    Direct Patient Care

    • Manages social work services for patients and their families
    • Supervises IDT patient plan of care for social work interventions
    • Conducts individual and family centered, thorough psychosocial assessment
    • Provides psychosocial interventions ensuring cultural appropriateness and developmental stage along the illness continuum, including:
      • supportive counseling
      • psychoeducation
      • anticipatory guidance
      • advocacy
      • facilitation of family meetings
      • connection to community resources
      • non-pharmacological pain and symptom management
    • Enhances patient and family coping as they navigate the challenges of illness
    • Offers clinical guidance and consultation to interdisciplinary teams
    • Delivers safe and comprehensive care coordination and discharge planning as required. Evaluates team coping with complex clinical cases. Provides guidance and debriefing when needed.
    • Conducts planned and unscheduled visits as part of the Interdisciplinary Team to conduct psychosocial assessment of patient/family functioning. Schedules and prioritizes visits based on the patient/family's acuity level. Conducts informational visits as requested by manager to potential patients/families.
    • Provides written psychosocial assessment of patient and family promptly, including psychosocial strengths and weaknesses, coping skills, grief reactions, support systems, high-risk factors for complicated bereavement, and the necessity for community resources.
    • Delivers individual and/or family counseling and reassesses psychosocial factors during each visit to ensure optimal results. Initiates or participates in patient care conferences or family meetings as required and completes proper documentation in the electronic record.
    • Upon assessing patient/family needs, makes all relevant referrals to other community agencies promptly; these may involve accessing and applications for financial resources, nursing home placements, etc.
    • As a Mandated Reporter, demonstrates awareness of family violence, neglect and abuse of children/elderly/disabled individuals to recognize those at risk and refer them to appropriate agencies under the Manager/Director's guidance. Demonstrates understanding of crisis intervention theory.
    • Runs or co-runs monthly support groups for IDT and assists with providing Bereavement Group support, as necessary.
    • Attends weekly IDT meetings to provide continual updates, planning, and adjustment of the POC as needed according to the patient/family's changing needs, and documents in biweekly updates. Participates in orientation, education, in-service training, and the QAPI [Quality Assurance Performance Improvement] program of Home Health.
    • Provides consultation to other healthcare personnel on psychosocial issues and delivers in-service education as requested.

    Documentation

    • Clinical visit notes validate the necessity for medical social services considering the patient's overall medical condition and experiences, the complexity of services rendered, and the plan for the next visit based on prior outcomes rationale.
    • Initial assessment must be finalized within 5 days of admission
    • Completes documentation following agency timeliness protocols.
    • Timely completion of IDT notes including risk assessment and follow-up plan

    Qualifications

    • Master's degree in Social Work (MSW) from an accredited school of social work.
    • Holds a current LCSW, and is licensed to practice in Massachusetts.
    • Preferred: Current Massachusetts Licensed Independent Clinical Social Worker (LICSW)
    • Maintains a valid American Heart Association (AHA) CPR certification
    • Minimum of 3 years of Medical Social Work experience.
    • At least 1-year experience in Home Health care.
