    POSITION FEATURES:

    PURPOSE AND SCOPE:

    The professional registered nurse Outpatient RN CAP 2 may be an entry-level designation into the Clinical Advancement Program (CAP) for new employees who meet the RN CAP 2 criteria or attained through advancement from RN CAP 1. This role is accountable and responsible for the provision and coordination of clinically competent care, including assessment, planning, intervention, and evaluation for an assigned group of patients. Responsibilities may involve delegating appropriate tasks to direct patient care staff such as RNs, LVN/LPNs, and Patient Care Technicians. As a member of the End Stage Kidney Disease (ESKD) health care team, this position actively participates in decision-making, teaching, leadership functions, and quality improvement activities that enhance patient care outcomes and facility operations.

    PRINCIPAL DUTIES AND RESPONSIBILITIES:

    • Performs all essential functions under the direction of the Supervisor and with guidance from the Clinical Educator, Preceptor or in collaboration with other Registered Nurses.
    • Conducts systematic collection and analysis of patient data pre - during - post hemodialysis treatment for assigned patients, makes adjustments to treatment plans as needed, and communicates with the healthcare team as necessary.
    • Assesses, collaborates, and documents patient/family's basic learning needs to provide education.
    • Directs and ensures safe and effective delivery of hemodialysis therapy to assigned patients.
    • Administers medications as prescribed, documents justifications, and effectiveness.
    • Initiates or assists with emergency response measures.
    • Serves as a resource and mentor for new and existing employees, setting examples of appropriate behavior and work habits.
    • Ensures correct laboratory procedures are followed and reschedules missed collections.
    • Identifies expected outcomes, updates care plans, and collaborates with the Interdisciplinary Team.
    • Raises patient awareness about treatment options.
    • Participates in education and quality improvement projects.
    • May serve as a Preceptor to new employees and complete CAP requirements.
    • Performs other duties as assigned.

    PHYSICAL DEMANDS AND WORKING CONDITIONS:

    • Direct patient care involves heavy lifting and assisting with ambulation.
    • Requires prolonged periods of standing and bending over.
    • May involve moving heavy equipment and exposure to contagious materials.
    • Work environment includes temperature control and moderate noise levels.

    SUPERVISION:

    • Supervises Patient Care Technicians/LVN/LPNs/RNs as a Team Leader or Nurse in Charge after meeting specific requirements.

    EDUCATION and LICENSURE:

    • Graduate of an accredited School of Nursing.
    • Current state licensure and CPR BLS Certification.
    • Minimum 2 years of Nephrology Nursing experience for RNs.

    EXPERIENCE AND REQUIRED SKILLS:

    • Entry level for RNs with minimum of 2 years of Nephrology Nursing experience.

    • Participates in quality improvement initiatives and mentors team members.

    • Conducts program and regulatory audits and recommends improvements.

    Clinical Advancement Program (CAP) Preceptor

    • Supervises new nurses, identifies learning opportunities, and collaborates with the team.
    • Communicates progress of new nurses and continues to support them.

    Clinical Advancement Program (CAP) Multi-modality Cross Trained

    • Trained in multiple modalities of patient care and maintains skillset for safe delivery of care.
    • Ensures patient satisfaction and compliance with company policies and procedures.

    Transitional Care

    • RNs with CAP 2 status participate in the Transitional Care Program, coordinating care for new dialysis patients and educating them on treatment options.
