We're looking for a Nurse Practitioner to join our team at Longevity Health.We provide collaborative primary care and services to a panel of members,
delivering ISNP Model of Care activities,
and developing individualized care plans for each member.
We facilitate Part A
& B services,
HEDIS & STARs quality gap closure,
- Serves as a trusted advisor to members,& loved ones,and nursing home staff.
- Promotes Longevity Health with members,& loved ones,&nursing facilities & other stakeholders.
