- Perform Adult/Geriatric Assessments, medical history review, diagnosis, treatment, and education
- Collaborate with healthcare professionals, patients, and caregivers
- Partner with Primary Care Physician for patient education and follow-up
- Deliver services in various settings including homes, facilities, and telehealth
- Master's Degree or equivalent experience with NP licensure
- Current RN and NP licensure with prescribing authority
- Board certification in a Matrix-approved specialty
- BLS, ACLS, or CPR certification
- Minimum 1 year experience as a Nurse Practitioner
- Strong computer skills and communication abilities
- Willingness to adapt to change
- Valid driver's license, reliable transportation, and adequate insurance
- Travel to patient locations as needed

Type: PRN/Part Time - Flexible-Nurse Practitioner
PAY: $90.00 or $95.00 (based on county) per Assessment
Location: Mount Gilead, Ohio
Hours: Flexible schedule including days, evenings, nights, and weekends
Benefits: Limited Benefits with 401(k) and Mileage Reimbursement
