Hospice RN Night Shift - On Call + Admissions (when needed)

James River Hospice is actively seeking a compassionate and highly committed Hospice RN to join our growing team in Fredericksburg You will be responsible for after hours support. This includes after hour calls, visits, providing emotional support and facilitating admissions as needed. As an integral part of our interdisciplinary team, you will ensure continuity of care and uphold our commitment to delivering compassionate end-of-life care. Brief Overview of Responsibilities:

• Complete an initial assessment of the patient and family to determine hospice needs
• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions.
• Provides coordination amongst the clinical team supporting the patient, and supervising LPNs and Hospice aids who may be involved in supporting patients.
• Spend quality time with the patient to assist with pain management and support
• Modify and re-evaluate the plan of care when necessary
• Monitor, record, and report symptoms and changes in the patient's condition
• Obtain and receive physician orders as required
• Participates in on-call duties as defined by the on-call policy
• Other duties as assigned

Qualifications:

• Must be a Registered Nurse with a current license to practice nursing in the Commonwealth of Virginia
• At least one year of hospice experience is required.
• Graduate of an accredited nursing school.
• Must have a valid driver's license and a reliable car available for use during work hours.
