    Join a patient-centered care coordination team where your expertise directly improves patient outcomes and healthcare navigation.

    This remote case management role focuses on short-term telephonic coordination, discharge planning, and patient advocacy—ideal for a professional who thrives in communication-driven care coordination and utilization review.

    Schedule:
    Telephonic case management with rotating weekend coverage every 7 weeks
    Caseload: 25–35+ patients managed over 30-day episodes with weekly follow-up calls

    Work Setting:
    Remote after in-office orientation (laptop and phone provided)

    Key Responsibilities:
    Conduct telephonic case management and care coordination
    Develop individualized care plans and monitor patient progress
    Coordinate multidisciplinary services and discharge planning
    Educate patients and families on treatment plans and available resources
    Advocate for patient needs and remove barriers to care
    Collaborate with healthcare teams to ensure seamless transitions

    Role Overview:
    This position requires strong communication skills, critical thinking, and the ability to manage multiple patients in a fast-paced environment. Experience in case management, discharge planning, and patient care coordination is essential.

    Requirements & Qualifications:
    Active RN license
    BLS certification
    Case management and care coordination experience
    Discharge planning experience
    Ability to work remotely with reliable home internet
    Knowledge of utilization review and insurance processes (preferred)
