- Perform Adult/Geriatric Assessments, medical history reviews, diagnosis and treatment, health education, physician referrals, case management, and clear documentation
- Collaborate with healthcare professionals, family members, and caregivers
- Work in various settings including Home Visits, Skilled Facility Visits, and Telehealth Visits
- Master's Degree required OR equivalent NP licensure experience
- Current RN and NP licensure in state of practice
- Board certified in a Matrix-approved specialty
- Current BLS, ACLS or CPR certification
- Minimum 1 year experience as a Nurse Practitioner
- Strong computer skills
- Excellent communication skills
- Comfortable with change
- Valid state driver's license and reliable transportation
- Travel to patient locations

Nurse Practitioner - PRN/Part Time - Home Risk Assessments
Type: PRN/Part Time - Flexible-Nurse Practitioner
PAY: $95.00 or $100.00 per completed Assessment
Location: Home, SNF, Telehealth and community environments in Burnet, Texas
Hours: Days, Evening, Nights, weekends, Flexible Scheduling
Benefits: PRN/Part Time providers receive 401(k) and limited Voluntary Benefits
