    Job DescriptionSalary: $40-$70 per visit

    Crystal Home HealthCare is looking for Occupational Therapy (OTR) Case Managers to join our team of healthcare professionals and make a difference, and we're inviting you to be part of our team We have open positions and are looking for the very best to support clients in their homes in the Metro Detroit Area.

    Qualifications

    • Graduate of an Occupational Therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association (AOTA) and/or certified by successfully completing the National Certification examination.
    • Currently licensed in the state of Michigan and in good standing as OTR
    • Two (2) years experience preferred.
    • Able to provide skilled therapy services in the home and able to instruct/teach patient and caregivers where necessary: comprehensive patient assessment including care planning and goal setting,
    • Able and willing to supervise COTAs providing services to the patient according to regulatory guidelines
    • Current & Valid Driver's License with reliable and insured transportation
    • Current CPR Certification
    • Current Negative TB test/negative chest x-ray
    • Functioning Electronic Device with cellular service so you can chart in the field
    • Knowledge and experience working with home health care specific Electronic Charting
    • Work requires knowledge of the Homecare Industry, Medicare/Medicaid/CMS/State of Michigan regulations

    Essential Functions:

    • Care for multiple patients throughout the day in their individual home environments
    • Perform and complete occupational therapy assessment including CMS OASIS documents
    • Report significant findings to physician and Clinical Manager
    • Implement physician orders in a timely, complete and accurate manner
    • Ensure the safety of the treatment procedures, patient and environment at all times
    • Supervise and evaluate the care provided by COTAs
    • Teach the patient and/or family members about new diagnosis, care, and support skills
    • Submit documentation in a timely, complete and accurate manner
    • Attend in-services and meetings as required
    • Maintain HIPAA compliance and patient confidentiality at all times
    • Position requires 100% travel to our patient's home
