    LCSW (Intrepid Spirit, Mental Health):Services shall be provided in the assigned clinical area Monday through Friday between the hours of for an 8.5, 9, or 10.5 hour shift which include an uncompensated 30 minute or one-hour meal break depending on length of shift.

    Unless services are required in accordance with this paragraph, in no instance will the HCW exceed 80 hours per 2-week period.

    NH YumaLCSW (PCBH/BHIP)Services shall be provided in the assigned clinical area Monday through Friday between the hours of 0600 and 1800 for an 8.5-or 9 or 10.5 hour shift inclusive of a 30 minute or 1-hour uncompensated meal-break depending on shift length.

    Unless services arerequired in accordance with this paragraph, in no instance will the HCW exceed 80 hours per 2-week period.


    Closures:


    During a planned closure of the facility due to training, holiday or unplanned closure due to unusual and compelling circumstances (e.g., natural disasters, military emergencies, severe weather), the Contractor willbe compensated for the unplanned closure.

    HCWs performing under this contract must be a U.S. Citizen as defined by the Department of Homeland Security, U. S.

    Citizenship, and Immigration Services, either a noncitizen national of the United State, lawful permanent resident of the United States, oralien authorized to work in the United States.


    Labor Category:

    Degree:
    Master's degree in Social Work (MSW).

    Education:
    Graduate from a School of Social Work fully accredited by the Council on Social Work Education (CSWE).

    Certifications:

    American Heart Association Basic Life Support (BLS) or American Red Cross Basic Life Support (BLS) for Healthcare Providers or American Heart Association or American Red Cross Healthcare Provider Course.

    HCWs who do not hold current certification must acquire certification prior to initiating contract performance. Web based classes do not meet these standards.

    Experience:
    Possess a minimum experience of two years within the last three years as a Licensed Clinical Social Worker.

    Licensure:
    Current, full, active, and unrestricted license to practice as a Licensed Clinical Social Worker.
    HCW Duties (5.

    1 Task 1): The duties for the HCW are as follows:


    • Coordinate appropriate referrals to community and other Government agencies for services.
    • Independently assess the psychosocial functioning and needs of patients and their family members and formulate and implement a treatment plan, identifying the patient's problems, strengths, weaknesses, coping skills and assistance needed, in collaboration with the patient, family and interdisciplinary treatment team.
    • Independently conduct psychosocial assessments and provide psychosocial treatment to a wide variety of individuals from various socio-economic, cultural, ethnic, educational, and other diverse backgrounds.
    • Use medical and mental health diagnoses, disabilities, and treatment procedures. This includes acute, chronic, and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology.
    • Implement treatment modalities in working with individuals, families and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals.
    • Independently provide counseling and/or psychotherapy services to individuals, groups, couples, and families. Must practice within the bounds of the license or certification.
    • Provide consultation services to other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment.
    • Participate in professional peer review case conferences, research studies, or other organizations required at the MTF

    Job Summary:


    Contractor personnel shall perform a full range of duties consistent with their labor category, education, training, experience, clinical privileges (as applicable), and assigned position.

    General duties that apply to all personnel or across certain labor categories are given below Maintain a level of productivity comparable with that of other individuals performing similar services Participate in peer review and performance improvement activities Practice aseptic techniques as necessary.

    Comply with infection control guidelines to include the proper handling, storage, and disposal of infectious wastes, and the use of universal precautions to prevent the spread of infection Function with an awareness and application of safety procedures Perform efficiently in emergency patient situations following established protocols, remaining calm, informing appropriate persons, and documenting events.

    Anticipate potential problems/emergencies and make appropriate interventions.

    Notify supervisor, director, or other designated person regarding problems that the HCW is unable to manage Apply an awareness of legal issues in all aspects of patient care and strive to manage situations in a reduced risk manner Participate in the implementation of the Family Advocacy Program as directed.

    Participation shall include, but not be limited to, appropriate medical examination, documentation, and reporting Exercise awareness and sensitivity to patient/significant others' rights, as identified within the MTF Maintain an awareness of responsibility and accountability for own professional practice Participate in continuing education to meet own professional growth Attend and/or comply with all annual training classes required by the Command, to include but not limited to online annual training provided by the MTF:

    disaster training, infection control, Sexual Harassment, Bloodborne Pathogens, Fire and Safety, Chemical, Biological, Radiological, Nuclear and Explosives (CBRNE), and all other required training Actively participate in the command's Performance Improvement Program.

    Participate in meetings to review and evaluate the care provided to patients, identify opportunities to improve the care delivered, and recommend corrective action when problems exist Participate in the provision of in-service training to clinic staff members.

    Provide training and/or direction as applicable to supporting government personnel (e.g., hospital corpsmen, students, etc Attend and participate in various meetings as directed Perform timely, accurate, and concise documentation of patient care Operate and manipulate automated systems such as CHCS, AHLTA, ADS, Essentris, MHS Genesis and Clinical Information System (CIS), participate in clinical staff Performance Improvement (PI) and Risk Management (RM) functions, as prescribed by theCommander.

    Maintain DOD email account as directed.

    The HCW shall be responsive to all email and voicemail communications Exercise appropriate delegation of tasks and duties in the direction and coordination of health care team members, patient care, and clinic activities Maintain documentation of all treatment provided in accordance with clinic directives and prepare such records and reports as may be required.

    All records and reports must be legible. Abbreviations must be only those listed in local instructions.


    Duties-Specific:

    Perform assessments, evaluations, consultations, differential diagnosis, and treatment for all disorders defined by the Diagnostic and Statistical Manual for Mental Disorders (current edition) published by the American Psychiatric Association Work closely with clinical staff and/or base personnel regarding patient awareness of mental health wellness programs (addictions, stress management, group therapies Provide therapeutic treatment and evaluation of patients in the Family Advocacy Program dealing with domestic violence, physical and sexual abuse, as requested Provide patient consultations requested by NHCP staff health care providers within guidelines established by the NHCP Provide therapeutic treatment (as applicable) and evaluation of patients with Post Traumatic Stress Disorder, Combat Stress Disorders and Traumatic Brain Injury (TBI).

    Conduct collaborative evaluations to facilitate treatment planning for TBIpatients Provide input to medical board determinations and administrative separations as requested Conduct clinical staff education when requested to enhance knowledge, understanding, and awareness of post-traumatic stress disorder, and treatment options for the management of post-traumatic stress disorder, traumatic brain injury and other combat related disorders.

    In lieu of the experience listed in the contract, the following additional or supplemental qualifications are required for HCWs on this TO:

    Provide two letters of recommendation written within the preceding two years from practicing providers, supervisors or program administrators attesting to the HCW's professional skills, competencies, patient rapport, training abilities, etc.

    Recommendationletters must include name, title, phone number, date of recommendation, address and signature of the individual providing the recommendation.

    The HCW shall provide services within the scope of their competencies and the applicable duties provided in the basic contract Provide therapeutic treatment (as applicable) and evaluation of patients with Post Traumatic Stress Disorder, Combat Stress Disorders and Traumatic Brain Injury (TBI).

    Conduct collaborative evaluations to facilitate treatment planning for TBIpatients Provide input to medical board determinations and administrative separations as requested Conduct clinical staff education when requested to enhance knowledge, understanding, and awareness of post-traumatic stress disorder, and treatment options for the management of post-traumatic stress disorder, traumatic brain injury and other combat related disorders LCSW, BHIP

    Deliver brief, consultation -based services to patients and PCMs using an integrated care model.

    Services provided are standard and mimic the structure, function, and pace of primary care (i.e., psychotherapy or other specialty care psychological assessments are inappropriate and therefore prohibited).

    The PCM is ultimately responsible for the patient's health care plan and makes the determination on how the HCW's recommendations might be integrated into the PCM's plan for the patient Provide timely and succinct feedback to primary care physicians regarding consultations findings and recommendations Determine the appropriateness of the patient receiving services in the primary care setting and assisting with implementation of treatment plans Formulate behavioral health interventions appropriate to primary care setting and assisting with implementation of treatment plans Provide brief follow ups, including relapse -prevention education Develop, teach, and provide clinical guidance BHIP for classes that promote education and skill-building to enhance psychological and physical health See approximately 8-14 patients a day Provide concise documentation of care and recommendations in patient's medical record in no less than 24 hours after the patient is seen Participate in group medical appointments including Shared Medical Appointments (SMA) and Drop-in Group Medical Appointments (DIGMA) as requested by the PCM Provide technical direction and clinical insight into relevant projects being conducted at the site of service Training requirement The HCW must successfully complete the Tri Service Internal Behavioral Health Consultant (IBHC) training course within 60 days of performance under the task order In addition, the HCW must meet the passing criteria for IBHC core competencies (i.e., scores a minimum of 3 out of 5 on DoD Core Competency Skills Form) as determined by a government expert trainer.

    This demonstration of IBHC core competencies may occur at any time after successful completion of the IBHC training If a HCW fails to pass the IBHC training or fails to demonstrate IBHC core competency, the contractor shall be required to provide a new HCW.

    The contractor shall not present the same HCW for another attempt.

    To promote continuity of services, the government will only provide IBHC training and travel funds for two completion attempts for the life of the requirement under the contract, inclusive of any subsequent logical follow-ons or option periods.

    If more than two completion attempts are necessary to comply with the task order requirements, the burden of cost for additional training and travel shall be borne by the contractor
