    Position Overview

    Join St. Luke's dedicated team of Registered Nurses (RNs) committed to fostering a positive, inclusive environment that ensures exceptional patient care experiences. Our home health professionals deliver comprehensive healthcare services in the comfort of patients' homes, catering to individuals of all ages. From infants to seniors, we utilize cutting-edge techniques and technology to support patients throughout their healthcare journey, facilitating smooth transitions from inpatient to outpatient care settings. Collaborating closely with St. Luke's Hospice team, our home-based services professionals (Nurse Practitioners, RNs, Medical Assistants, Social Workers) oversee patient care transitions.

    The Home Care Coordinator collaborates with physicians and interdisciplinary team members to create and oversee individualized care plans from admission to discharge for an assigned caseload. This role involves providing direct in-home care requiring travel within the Treasure Valley region.

    Expectations for this Role

    • Full-time position with flexible scheduling (Monday-Friday 8-hour shifts or Sunday-Thursday 10-hour shifts) including rotating evening calls and weekends
    • Manageable caseloads with daily visit requirements below state and national averages
    • Compensation for drive time and mileage reimbursement in the Treasure Valley region
    • Potential for Differential and On-call Pay

    Requirements

    • Education: Nursing Degree
    • Experience: Minimum of 2 years RN experience, with Hospice or Acute Care background preferred
    • Licenses/Certifications: Current RN License in the State of Practice, Current Basic Life Support (BLS) Certification Provider Certified through American Heart Association or American Red Cross, Valid Driver's License, and Auto Insurance
