    Job Title: HOME CARE PHYSICAL THERAPIST

    At Liberty Home Care, we understand the importance of recovering at home after an illness, trauma or surgery. Our healthcare professionals are dedicated to providing independent recovery for our patients.

    We are seeking a skilled:

    • Provides patient care services and direct treatment with follow-up.
    • Evaluates new patients with physician orders and develops a plan of care based on diagnosis.
    • Documents patient care data accurately, thoroughly and in a timely manner, reflecting evidence of improvement/progress/decline in patient condition.
    • Submits daily/weekly patient visit schedules to the Scheduler.
    • Communicates as necessary with intercompany and intracompany contacts.
    • Discusses reimbursement policies and treatment plans with patients/families and Patient Care Coordinators.
    • Coordinates referrals to appropriate disciplines.
    • Provides in-service education, healthcare professional education and public education, supervising peers and supporting personnel/students as needed.
    • Conducts research and assists with employee orientation.

    Requirements:

    • Bachelor's degree in physical therapy from an accredited school and current licensure in the state where therapy is provided.
    • Prior experience in a non-home healthcare environment preferred.
    • Strong knowledge of physical therapy theory and concepts, with ability to evaluate and treat patients with physician orders.
    • Minimum 12 hours of continuing education annually.
    • Acceptance of assigned travel and working conditions.

    Salary: $80,000 - $120,000 per year, depending on experience.
