- Patient Care Coordination:
- Assess patient/support system needs for resources based on medication management, psychotherapy, ongoing medical care, housing, financial support, community support, and county-based resources.
- Ensure discharge planning begins at admission and obtain collateral input from providers and supports.
- Transition and Discharge Planning:
- Develop and implement transition, discharge, and aftercare plans prior to discontinuation of behavioral health services.
- Schedule aftercare appointments for patients with PCPs, psychiatrists, next-level therapy programs, living arrangements, etc.
- Contact referral sources and/or other treatment providers within designated time frames and document accordingly.
- Therapeutic Services:
- Provide cognitive behavioral therapy, dialectical behavior therapy-focused, behavioral activation, interpersonal psychotherapy, and other group and individual therapeutic services to patients.
- Implement goals, objectives, and interventions as specified in the patient's individual treatment plan.
- Milieu Management:
- Maintain safety of the area and provide a structured therapeutic milieu where positive patterns of social, educational, and personal behavior are taught, reinforced, and enhanced.
- Professional Development:
- Demonstrate organizational skills, effective communication, and punctuality.
- Participate in training, in-services, seminars, and performance improvement teams to increase knowledge and awareness of regulations.
- Bachelor's degree in human services or equivalent strongly preferred.
- State appropriate substance use credential required or obtained within 90 days of hire.
- Minimum of two years' work experience serving persons with mental health and substance use disorders.
- American Heart Association Healthcare Provider CPR certification or American Red Cross Professional Rescue required within thirty days of date of hire.
