    Nurse Practitioner, Family Medicine

    About Role


    At Vera Whole Health, we are searching for a compassionate Nurse Practitioner in Family Medicine. In this role, you will have the opportunity to establish meaningful connections with your patients, going beyond just treating symptoms. With a daily schedule of meeting 10 to 15 patients for appointments ranging from 30 to 60 minutes, and supported by a dedicated care team, including a health coach, you will be delivering comprehensive care with a focus on the whole person - a unique approach in today's healthcare system.

    Essential Functions


    • Embrace Vera Whole Health's mission, vision, and values
    • Deliver evidence-based primary care to a set number of patients each day
    • Collaborate with patients and care team to create comprehensive care plans
    • Ensure alignment with the Vera medical home model in providing comprehensive care
    • Assess patient needs, promote participation in health evaluations, and health promotion activities
    • Provide clinical leadership and support organizational activities
    • Work towards achieving established clinical objectives and health outcomes
    • Rotate for after-hours coverage as required

    Education & Experience


    To qualify for this role, you should possess:
    • MD, DO, or NP Board Certification in primary care
    • Current and valid medical license along with DEA certification
    • Minimum 3-5 years of primary care experience
    • 2+ years of case management experience with a focus on prevention and population health
    • Experience in team-based care and EMR systems
    • Proficiency in coding, documentation, and Medicare Advantage
    • Familiarity with Google Workspace
    • Bilingual Spanish/English proficiency is a plus

    Compensation: $115k-$138k annually (may vary based on market)
