    JOB SUMMARY

    As a Case Manager, you will be responsible for delivering top-notch in-home case management services to Veterans and families participating in the Rapid Rehousing & Shelter Diversion Program. Your role involves helping clients set and achieve housing stability goals, offering support, guidance, and connecting them with essential services to ensure they maintain permanent housing and reach their life objectives.

    PRIMARY RESPONSIBILITIES
    • Develop individualized Housing Stability Plans for clients focused on financial needs, budgeting, employment, educational services, family support, and community living.
    • Provide community outreach, mediate tenant-landlord issues, and prevent homelessness.
    • Connect Veterans with healthcare services and assist in health plan adherence.
    • Offer life skills counseling and support.
    • Conduct regular in-home visits, monitor client progress, and facilitate necessary linkages.
    • Manage Temporary Financial Assistance processes.
    • Collate and report monthly statistics to the Director.
    QUALIFICATIONS
    • Minimum Bachelor's Degree in Human Services or a related field.
    • 2 years of successful experience working with homeless or underserved populations.
    • Flexibility to work weekends and evenings.
    • Respect for confidentiality and diversity.
    • Strong teamwork and communication skills.
