- Conduct thorough assessments of adult patients' health status, considering physical, emotional, and psychosocial factors.
- Collaborate with patients, their families, and interdisciplinary team members to develop individualized care plans tailored to meet the unique needs and preferences of each patient.
- Continuously reassess patients' conditions and adjust care plans as necessary to ensure optimal outcomes and quality of life.
- Provide skilled nursing care to adult patients, including medication administration, wound care, and management of chronic conditions.
- Monitor patients' vital signs, symptoms, and response to treatment, and communicate any changes or concerns to the healthcare team.
- Coordinate with physicians and other healthcare providers to ensure seamless continuity of care and timely interventions as needed.
- Assist adult patients with ADLs, such as bathing, grooming, dressing, and mobility, while promoting independence and dignity.
- Provide guidance and support to patients and their families on adaptive techniques and assistive devices to enhance self-care and mobility.
- Foster a supportive and empowering environment that encourages patients to participate in their own care and rehabilitation efforts.
- Educate adult patients and their families on disease management, medication adherence, and lifestyle modifications to promote health and wellness.
- Provide emotional support and counseling to patients coping with chronic illnesses, disabilities, or end-of-life issues, fostering a compassionate and empathetic approach.
- Address any questions or concerns raised by patients or their families, and empower them to make informed decisions about their healthcare options.
- Maintain accurate and thorough documentation of all patient assessments, interventions, and outcomes in compliance with regulatory standards and agency policies.
- Communicate effectively with interdisciplinary team members, including physicians, therapists, and social workers, to coordinate care and facilitate care transitions.
- Participate in case conferences and care meetings to review patient progress, discuss care plans, and identify opportunities for improvement.
- Current RN license in the state of practice.
- Certification as a Home Health Aide (CHHA) preferred.
- Minimum of 6 months years of clinical experience in nursing required.
- Strong assessment, clinical judgment, and problem-solving skills.
- Excellent communication and interpersonal skills, with the ability to build rapport and establish trusting relationships with adult patients and their families.
