Job Summary:

The main responsibility of a Medical Social Worker II is to support patients and families in handling the emotional and practical aspects related to the illness. Reporting to the Social Work Manager/LCSW, the role involves providing age-appropriate social work care, conducting assessments, planning treatments, and offering information and assistance to meet the diverse needs of patients and families in healthcare settings.

Key Responsibilities:

• Offering psychodynamic interventions, crisis support, counseling, and developing coping strategies.
• Developing individual treatment plans to help patients and families cope with various factors affected by the illness.
• Completing psychosocial assessments and creating treatment plans based on patient needs.
• Discussing care options and resources with patients and families proactively.
• Collaborating with healthcare team members to provide comprehensive services to patients.
• Advocating for patients to receive appropriate services.
• Assisting with care planning and discharge plans.
• Reviewing and prioritizing referrals, maintaining records, and documentation.
• Coordinating care placement for Kaiser members in nursing facilities.
• Acting as a liaison between patients, Kaiser, and other facilities.
• Identifying suitable care levels and facilities for patients.
• Collaborating with internal and external resources to achieve common goals.
• Providing information and referrals to community resources.
• Participating in quality assurance activities and coordinating communication.
• Handling necessary clinical and psychosocial information for referrals.
• Performing other duties as required.

Grade 10

Basic Qualifications:
Experience

• Step I: 0 - 2 years of social work experience.
• Step II: 2 - 4 years of social work experience within the past five (5) years.
• Step III: 4 or more years of social work experience within the past ten (10) years.

Education

• Masters in Social Work accredited by the Council of Social Work Education.

License, Certification, Registration

• N/A

• Demonstrated ability to work collaboratively in a team.
• Strong psychosocial assessment skills.
• Knowledge of chronic and acute diseases impact on patient and family functioning.
• Excellent communication and documentation skills.
• Proficiency in clinical information systems.
• Capacity to handle a demanding workload efficiently.
• Ability to work independently and as a team player.
• Ability to understand complex benefit information and care coordination.
• Willingness to work in a collaborative environment.
• Refer to the Position Specifications in the bargaining agreement.

Preferred Qualifications:

• At least one (1) year post MSW experience in a health care setting preferred, including consideration of an MSW internship.
• LCSW preferred.
