Overview
The Telehealth Advance Practice Provider:
> Uses MyChart e-visit a technology to formulate clinical decisions to manage non-emergent illnesses.
> Utilizes patient-generated questionnaires and patient history to conduct care planning.
> Integrates professional experience, education, and leadership into the assigned clinical role.
> Functions and promotes an environment of collegial relationships with all clinical and nonclinical roles involved in the delivery of the e-visit program.

Responsibilities
Complete documentation that meets regulatory and industry standards. No documentation should denote reasons for delayed responses as this is the patient?s medical record.
Complete written communication for patients, including education on current condition and care.
Follow established policies and protocols to recommend and assist with transfer to a higher level of care if needed.
Identify, assess, and coordinate the treatment of e-visit patients consistent with best practice standards of care.
Recommend treatment plans and advise against potential complications within 1 hour of submission of e-Visit request.
Maintain performance standards, service excellence, and operational efficiency at all times.
Ensure adoption and utilization of best practice provider service standards and methodologies.
Maintain responsibility for the diagnosis, treatment, and clinical management of patients through an electronic format.
Meet productivity metrics pertaining to turnaround times and visit duration times.
Obtain, review, and utilize the electronic medical record to assess, diagnose, and treat conditions remotely.

Qualifications
EDUCATION:
REQUIRED:
Advance Degree as Nurse Practitioner or Physicians Assistant from an Accredited Program
EXPERIENCE:
REQUIRED:
2 year of experience as a Nurse Practitioner or Physicians Assistant providing care in a Primary Care Setting
PREFERRED:
3 years of experience as a Nurse Practitioner or Physicians Assistant providing care in a Primary Care Setting
LICENSURE/CERTIFICATION/REGISTRY/
