    Employment Status:

    0.9 FTE (Full time Equivalent) / 72 Hours Per Pay Period

    Hours:
    • Monday to Friday: 8:00am to 4:30pm
    • No Weekends
    Position Objective

    The Discharge Planner provides services to patients facing substantial and often challenging medical issues. The discharge planner will also be responsible for the adequate discharge planning for all patients referred to them by the nursing service and/or Physicians in the hospital. The discharge planner will be guided in this process by the Discharge Planning Program that has been established at the hospital. Referrals may be given for needs, other than discharge planning, and it will be the responsibility of the discharge planner to follow through with appropriate referrals and arrangements of services. The discharge planner is under the general supervision of the Social Services Supervisor and shall allowed considerable freedom in the fulfillment of responsibilities.

    Essential Responsibilities
    • Conduct and document assessment of patient and family needs, communicate care issues, and develop appropriate plan
    • Perform disposition assessments and discharge planning for high risk patients
    • Refer patients to appropriate community resources
    • Provide consultation to Physicians and hospital staff regarding psychosocial issues, bioethical issues, and discharge problems
    • Document pertinent information regarding discharge plans in the hospitals electronic record system
    • Attend daily Multidisciplinary meetings and ongoing daily communication with Physicians
    Other Responsibilities
    • Perform all other related duties as assigned in a professional and responsive manner
    • Demonstrate the ability to use relevant equipment
    • Support, understand, and promote Alomere Health's mission, vision, values, policies and procedures
    • Ability to meet the work schedule requirements with flexibility dependent upon the needs of the department

    Required Qualifications
    • Bachelor's Degree in Social Work OR Licensed Practical Nurse (LPN)
    • Licensed in the State of Minnesota
    Preferred Qualifications
    • Prior hospital experience
    Knowledge, Skills and Abilities
    • Ability to establish relationships- Position is required to establish good working relationships with Medical Staff Agencies as well as patients, relatives, friends, co-workers, and Physicians
    • Ability to establish a strong working relationship with the Clinical Directors and Charge Nurses of each unit of the hospital in order to ensure our patients are adequately supported in their for planning relative to discharge and other needs
