We are committed to providing crucial behavioral health services and supportive housing options to those experiencing homelessness in Multnomah County. As part of our team, you can truly make a difference in the lives of those who require assistance.

    The PATH Street Outreach Specialist is key in linking individuals experiencing homelessness with PATH (Promoting Access to Hope) services and other community service providers. This role involves identifying and engaging communities that have not traditionally sought drug and alcohol services. By providing in-person support, you will address care gaps, offer health education, collaborate with partners, and develop strategies to meet community needs, all while maintaining a culture of respect and understanding for the diverse populations served by the Health Department.

    This is a Limited Duration assignment, with a maximum duration of two years.

    As a PATH Street Outreach Specialist, you will:

    Outreach and Direct Client Services

    • Engage with individuals experiencing street homelessness and marginalized populations with respect
    • Support clients in addiction recovery and behavioral changes
    • Provide information on community resources and health education events
    • Collaborate with partners to connect clients with necessary services

    Team-Based Care

    • Build effective relationships with team members and communities
    • Participate in case conferences and maintain communication with stakeholders
    • Display empathy and understanding towards individuals from diverse backgrounds

    To Qualify:

    We consider a combination of relevant work experience, education, and transferrable skills for this position.

    Required Minimum Qualifications/ Transferable Skills*:

    Education/Training: Associate's Degree and 1-4 years of experience

    Certification: Basic Life Support (BLS) CPR Certification

    Other: Valid Driver's license and willingness to undergo a background check

    Preferred Qualifications:

    • Active Certified Recovery Mentor (CRM) certification
    • Demonstrate equity, intersectionality, and anti-racist practices
    • Advanced knowledge of communities experiencing marginalization

    Screening & Evaluation:

    We will review applications based on minimum qualifications, resume, cover letter, and assessments.

    Type of Position: Represented, eligible for overtime

    FLSA: Non-exempt

    Salary/Pay: Compensation aligned with Oregon Equal Pay Law
