    Job Summary:


    Under general supervision, instructs, counsels, monitors and evaluates nursing personnel during orientation to Agency ;assists in planning, presenting, evaluating and revising staff development activities; performs duties of team leader or Staff Nurse when needed.

    Job Responsibilities:


    • Instructs, counsels and monitors the work of nursing orientees in the aspects of clinical nursing relative to assigned area; educates nursing orientees concerning nursing policies and procedures.
    • Collaborates with Nurse Educator/DOCS/Administrator in planning and structuring orientation program to meet the individual orientee's needs; plans daily assignments for orientees.
    • Performs on-going assessment and regular formal evaluations of nursing staff during orientation period; documents all observations and submits recommendations to supervisor (e.g., duration of individual orientation).
    • Assists Nurse Educator/DOCS/Administrator in planning, presenting, evaluating and revising the staff development activities and nursing policies and procedures.
    • Serves as a role model and resource person for all nursing staff.
    • Performs duties and responsibilities of charge nurse/team leader or Staff Nurse as needed or directed.
    • Adheres to the mission, vision, values and code of Business Conduct as established by Agency. Demonstrates an understanding of and a commitment to customer service values of respect, courtesy, ownership, privacy, professionalism and responsiveness.
    • Adheres to Agency and department policies, procedures, and objectives and perform sother duties as assigned.
    • Perform On call – Rotation weekends and do Kinsser Charting on timely manner. Provides training and guidance to assigned personnel.
    • Provides functional guidance and training to less skilled personnel.
    • May act as charge nurse or team leader.
    • May supervise nursing students.
    Experience:


    • Must have the clinical knowledge and critical thinking ability to effectively plan and provide coordination of patient care consistent with standards and regulations.

    • Must have exceptional interpersonal and customer services skills; must be able to effectively solve unique problems as they arise or identify when to consult the director.
    ·
    • Must have knowledge of current home health care admission criteria, Medicare, Medicaid, State and Federal regulations.
    ·
    • Must be a licensed RN by the State Board of Nursing.
    ·
    • Have had two (2) years of experience with a home health required.

    Requirements:


    ADDITIONAL DESIRABLE QUALIFICATIONS

    • Must be proficient in the use of Microsoft programs for internal communications (email) and database tracking/reporting, as well as proficiency working with electronic health records.
    • Must be proficient in the use Kinsser/WellSky Software.
    • Must have excellent oral and written communication skills and a strong commitment to customer service.
    • Skills
    :


    • Evidence of progressive leadership responsibilities, including fiscal management is required.
    • Demonstrated knowledge of home health licensure, regulations, and Medicare Conditions of Participation (COP).
    • Excellent communication skills.
    • Excellent interpersonal skills necessary in order to communicate effectively with a wide variety of personnel when identifying and resolving sensitive and complex patient or administrative issues.
    • Nursing skills as defined as generally accepted standards of practice.
    • Excellent interpersonal skills and ability to communicate effectively.
    • Demonstrates proven decision-making skills.
    • Must read, write and comprehend English.
    Education:


    • Bachelor's Degree in health-related field
    Licensure/Certification:
