- Actively engages in the agency's care coordination process including promptly reporting patients' needs and changes in condition, participating in patient case conferences, effectively communicating with the patient, family, physician, other healthcare professionals, agency management staff, and case coordinator, community case managers (if applicable)

Job Summary
The Home Care Licensed Practical Nurse is accountable for delivering direct nursing care as instructed by the physician to acutely ill and/or chronically ill patients in their residences, and documenting clinical and progress notes; along with coordinating other patient care activities/services to offer the utmost quality of patient care in the assigned geographic service area. They operate under the immediate guidance of the Registered Nurse and/or assigned supervisor. Nursing responsibilities are restricted to those in accordance with the State LPN/LVN Practice Act, any relevant licensure/certification prerequisites, and agency protocols and processes.
Specific Job Duties/Responsibilities
License Requirements
Must possess current LPN licensure in the state of practice.
Current driver's license, vehicle insurance, and access to a reliable vehicle or public transportation.
Current CPR certification is mandatory.
Experience Requirements
Prior home care experience is preferred
Additional State Specific Requirements
LA: Current LPN licensure in LA without restrictions. Minimum of one year of LPN work experience.
